For Your First Visit
After you have scheduled your consultation and have received your security ID code from our office, please click the “Create An Account” link to create your account in our patient portal. Once logged in, please fill out your Patient Information and Medical History forms. Before you come in for your visit, please download and read our privacy policy, linked below as a pdf.
