Q: I am 56 years old and have been using propecia for 11 years. Over the last year I have noticed that it is losing some of it's effectiveness. I would like to switch to .5 Adovart or increase my dosage to 2mg of propecia. If I were to attempt this do you think there would a large shedding phase or would it be more seamless. Also would it be possible to introduce the Adovart gradually over a 1 to 2 months, alternating it with the Propecia

Dr. Bernstein: I usually advise finasteride 5mg 1/3 tablet a day as a next step. Usually the transition is seamless, but occasionally we do see shedding, even at this increment. There are many ways to step up the dose. As your question implies, the more gradual the increase, the less likely there will be shedding. I prefer to increase gradually so that there is more wiggle room to increase the dose as tolerance develops.
